Registration of the Death

Registration of the Death The Medical Certificate of Cause of Death can usually be collected from the surgery or hospital the day after bereavement occurs unless the death is sudden or unexpected.

This certificate must be presented at the relevant sub-district registrar’s office where the death occurred. We are able to offer the following contact details for registrars within the local Hertfordshire area:

The registrar will require the following information and documentation:

  • The Death Certificate
  • Full Name of the Deceased
  • Date and Place of Death
  • Occupation
  • Husband’s Occupation (Where Applicable)
  • Residence of Deceased
  • Place and Date of Birth of the Deceased

The registrar will issue the following certificates

  • The Green Disposal Certificate (to be given to J&S Funeral Service Ltd)
  • The Death Certificate (Retain for Banking and Insurance Purposes)
